For your convenience, here are the main rules and terms that payment cardholders should be aware of, also, application and notification forms that may be necessary to you as a cardholder.
General Service Rules of SEB Bank (PDF, 384 KB). This document should be read by everyone willing to have a payment card. It specifies in detail the terms that one should know as well as the cardholder’s rights and duties. You will find out about payment card issuance, their use and what should be done in case of a card loss, etc.
Lost Card Notice (PDF, 114 KB, in Lithuanian). This notice will be needed if you loose your card. Please fill it out and submit at any branch office of SEB Bank – we will fix everything for you, and a new card will be issued at your request.
An application for debit card(s) (PDF, 469 KB, in Lithuanian). The application will be required if you are willing to get a debit card. Print, fill out and submit the application at any unit of SEB Bank. Such application may be filled out and submitted via the Internet.
An application for debit card(s) for children aged 7 to 14 (PDF, 670 KB, in Lithuanian). The application will be required if you are willing to get a debit card for children aged 7 to 14. Print, fill out and submit the application at any unit of SEB Bank.
An application for credit card(s) or for change in credit limit terms (PDF, 2.1 MB, in Lithuanian). The application will be required if you are willing to get a credit card or change your credit limit terms. Print, fill out and submit the application at any unit of SEB Bank. Such application may be filled out and submitted via the Internet.
An application for additional card(s) (PDF, 720 KB, in Lithuanian). The application will be required if you are willing to get an additional payment card linked to an existing account. Print, fill out and submit the application at any unit of SEB Bank. Such application may be filled out and submitted via the Internet.
Application to change the statement of account delivery method (PDF, 107 KB, in Lithuanian). Card account statements are sent by ordinary mail, e-mail, or are made available via the bank's Internet services system. If you want to change the statement of account delivery method or to receive no statements at all, you should fill out the application and submit it to the branch office of SEB Bank.
Application to send a monthly statement of account by e-mail (PDF, 134 KB, in Lithuanian). If you prefer to receive monthly card account statements by e-mail or if you are willing to change the e-mail address for sending the statements, please fill out the application and submit it to any branch office of SEB Bank.
Description of the Insurance Contract Terms and Conditions (PDF, 125 KB, in Lithuanian). Holders of some credits cards issued by SEB Bank are provided with foreign travel insurance free of charge. This document specifies in detail in which cases one is eligible to the insurance benefit and in what amounts.
