A notice about the insured event can be submitted at any unit of the bank

In certain cases, e.g., if you are not SEB Internet bank user or do not have qualified signing app "Smart-ID", or mobile signature, you can submit a notice about the event at any unit of the bank.

Documents proving the event

  • Death certificate
  • Document proving the death reason, i.e. expertise of the course of illness or an extract indicating the cause of death and precise diagnosis
  • Other documents related with the insured event, i.e. insured’s medical records book and documents of the medical tests
  • If the insured died outside the territory of the Republic of Lithuania – the documents proving repatriation of the body through the state border
  • If the legal prevention institutions performed an investigation related to the insured’s death – conclusions of the legal prevention institutions
  • If a beneficiary is not nominated – the documents proving inheritance of the insurance benefit.
  • Death certificate
  • Medical documents from the healthcare institution including the approved diagnosis, anamnesis, results of the laboratory tests and  medical testing equipment, description of the prescribed treatment.
  • Decision of the Disability and Working Assesment Office under the Ministry of Social Security and Labour Republic of Lithuania regarding determination of  invalidity and incapacity to work.
  • A document have been issued by the medical institution stating the full diagnosis and the date of the diagnosis, applied treatment, full medical history.
  • Other documents related to the insurance event, if available at your disposal, e. g., investigation documents of legal entities, forensic or other examination conclusions and similar.
  • A document issued by a medical institution stating the full diagnosis, the date of event, the date of the diagnosis, the treatment used, the medical history.
  • X-ray photographies, tomograms
  • Expertise conclusions
  • Other documents related to the insurance event, if available at your disposal, for example, investigation documents, judicial documents.
  • Extracts from the medical documents about the injury circumstances, diagnosis, prescribed treatment and consequences
  • X-ray photographies, tomograms
  • Approved copies of the entries in the medical documents made by a radiologist
  • Expertise conclusions
  • Photos (if you submit pictures of any scars, other damage to the skin surface, the ruler should be visible next to an item of evidence being photographed enabling to measure the size of such scar or any other damage to the skin)
  • Other documents proving the accident (trauma).
  • In accordance with the procedure established by laws of the Republic of Lithuania a certificate of incapacity for work issued by a medical institution as well as a decision of Sodra to grant sickness benefit during a period of incapacity for work, or a certificate of Sodra on paid benefits.
  • A detailed medical record issued by the medical institution, certifying the fact and date of the accident, stating the final diagnosis, the treatment applied an its course and the result of the treatment.
  • A detailed description of the tests supporting the diagnosis as well as the radiological examination data. A detailed extract of medical records from an inpatient treatment facility if hospital treatment was required.
  • If available, please to provide other documents related to the insured event, such as documents of an investigation conducted by law enforcement authorities, conclusion of forensic and / or other examinations, etc.
