1. Submit a claim online

Submitting the notice form online takes only a couple of minutes

2. Submit documents proving the event

After submitting the claim note, please collect and provide medical and other documents to prove the event:

  • Medical death certificate
  • A document confirming the cause of death 
     - i.e. medical document about the course of illness or the course of death and precise diagnoses (if this information is missing on medical death certificate)
  • Other documents related to the insurance event
     - i.e. insured’s medical record, forensics reports, conclusions of expertice, police and prosecutor's office, etc
     - if a beneficiary is not nominated, please add inheritance documents (acceptance, certificate of inheritance)
  • Medical death certificate
  • Documents from the medical institution 
     - including the final diagnosis and its date, detailed medical history, diagnostic and medical tests results, prescribed treatment
  • Decision of invalidity and incapacity to work
    - issued by the Disability and Working Assesment Office under the Ministry of Social Security and Labour Republic of Lithuania 
  • Documents from the medical institution 
     - including the final diagnosis and its date, detailed medical history, diagnostic and medical tests results, prescribed treatment
  • Other documents related to the insurance event
     - investigation documents of legal entities, forensic or other examination conclusions and similar
     
  • Documents from the medical institution 
     - confirming the accident and exact date and final diagnoses
     - extract from the medical documents (epicrisis, ER reports, e-health records, etc) 
     - tests confirming the diagnoses (X-Ray, CT, MRI, Ultrasound, laboratory tests, etc), the treatment used
  • Other documents related to the insurance event
     - for example, investigation documents, judicial documents, etc
  • Documents from the medical institution 
     - confirming the accident and exact date and final diagnoses
     - extract from the medical documents (epicrisis, ER reports, e-health records, etc) 
     - tests confirming the diagnoses (X-Ray, CT, MRI, Ultrasound, laboratory tests, etc), the treatment used
  • Other documents related to the insurance event
     - for example, investigation documents, judicial documents, etc
  • Decision of Sodra to grant sickness benefit during a period of incapacity for work
    - sickness benefit due to an accident
  • Documents from the medical institution 
     - confirming the accident and exact date and final diagnoses
     - extract from the medical documents (epicrisis, ER reports, e-health records, etc) 
  • Other documents related to the insurance event
     - for example, investigation documents, judicial documents, etc
  • Treating doctor's report proving the disability is caused by illness or physical injury
  • The insured person’s health record, extracts from the medical documents, and copies of the tests performed with regard to the injury, bodily harm or illness 
  • If the insured person underwent medical treatments at the hospital within the period of coverage (specified in the application for the insurance benefit) – extracts from medical documents issued by the medical treatment institution
  • Other documents related to the insured event, if available
  •  If a traffic accident occurred – decisions of investigation institutions or police certifications, if available

If a sickness allowance is allocated and paid to the insured person by the territorial units of Sodra:

  • Spreadsheet available on the Electronic Citizen Service System (hereinafter referred to as EGAS) of Sodra, including the data of the electronic sick leave certificates issued to the insured person and notifications regarding the sickness allowance allocation
  • The insured person’s health record and originals or approved copies of the tests performed with regard to the injury, bodily harm or illness
  • Other documents related to the insured event, if available (e.g. investigation documents, forensic or other examination conclusions)
  • If a traffic accident occurred – decisions of investigation institutions or police certifications, if available 

If a sickness allowance is allocated and paid to the insured person by other entities than the territorial units of Sodra:

  • Medical certificate for absence from work, labour exchange or educational institution (form No. 094/a)
  • The insured person’s health record and originals or approved copies of the tests performed with regard to the injury, bodily harm or illness
  • Other documents related to the insured event, if available (e.g. investigation documents, forensic or other examination conclusions)
  • If a traffic accident occurred – decisions of investigation institutions or police certifications, if available 

For self-employed persons, unemployed persons and persons temporarily working outside of Lithuania:

  • Extract from the medical documents issued by the personal healthcare institution offering inpatient personal healthcare services
  • The insured person’s health record and originals or approved copies of the tests performed with regard to the injury, bodily harm or illness
  • Other documents related to the insured event, if available (e.g. investigation documents, forensic or other examination conclusions)
  • If a traffic accident occurred – decisions of investigation institutions or police certifications, if available

Upload the documents after you submit the claim

Upload the documents on the SEB internet bank under "Documents certifying the insured event".
Sign the documents with Smart-ID or mobile signature.
If you don't have access to the internet bank, Smart-ID, or mobile signature, you can submit the documents at any SEB branch office.

3. Receive an insurance benefit

After we receive your documents, we will get to work on your notice form as soon as we can. On average, it takes us 10 working days to make a decision. In more complex cases, it could take up to 30 days.
